Confidentiality
By accepting these terms, I consent that Hypnotherapy and Coaching Australia Pty Ltd
may release information to a specific individual or agency if it has been determined that a child, elder or any other person including a staff member of Hypnotherapy and Coaching Australia Pty Ltd is at risk or is currently being abused; If I as a client am in imminent danger to myself or others or if a subpoena of records has been requested. I also understand that at anytime, Hypnotherapy and Coaching Australia Pty Ltd may discuss aspects of my case with other colleagues keeping my full name and identity completely confidential unless I have given permission otherwise.
By booking a session with you hereby release Hypnotherapy and Coaching Australia Pty Ltd
from any liability or claims that could be made against any member of staff at Hypnotherapy and Coaching Australia Pty Ltd concerning mental and or physical wellbeing during the work that has been outlined and agreed upon now and in the future, by accepting the terms of use.
Liability
You agree that, except for death and personal injury arising from our negligence, we shall not be liable in contract, tort, negligence, statutory duty or otherwise, for any loss or damage whatsoever arising directly or indirectly from or in any way connected with the site or your use of or reliance upon the content or any information you obtain by means of the site or any bookings or purchases you make of goods and/or services through your use of the site, including, without limitation, damage for loss of profits, loss of business, or any other consequential or pecuniary loss (even where we have been advised of the possibility of such loss or damage.)
Scope of Practice
I understand that Hypnotherapy and Coaching Australia Pty Ltd is not a Medical Practitioner or Psychologist of any kind and that hypnosis should not be considered a replacement for the advice and or services of a Medical Doctor, Psychiatrist or Psychologist. Hypnotherapy and Coaching Australia Pty Ltd holds the right to refuse treatment, to cease treatment and refer me on at any time.
Participation
I give Hypnotherapy and Coaching Australia Pty Ltd full permission to hypnotise me and to use a combination of modalities methods, knowing that by my full participation in the process and by listening to my personalised recordings. I play a very significant role in my overall success of the treatment.
Guarantee
I understand that although attending a minimum of 4 consecutive weekly sessions with Hypnotherapy and Coaching Australia Pty Ltd there is a high success rate. Hypnotherapy and Coaching Australia Pty Ltd cannot and does not guarantee results since my own personal success depends on many factors that Hypnotherapy and Coaching Australia Pty Ltd
has no control over, including my willingness and desire to affect the changes inside of myself.
Audio Recordings
I give full permission to Hypnotherapy and Coaching Australia Pty Ltd to make audio recordings that may include my voice. I understand that if a recording/s are made during or after my session/s, Hypnotherapy and Coaching Australia Pty Ltd retains full copyright over any copies distributed to me.
